JOB SUMMARY/RESPONSIBILITIES: • Implements, maintains and provides clinical pharmacist services including: clinical, educational, informational and dis! tributional functions for newborn, infant, pediatric, adolescent, adult and geriatric patients of The Queen’s Medical Center (QMC). II. TYPICAL PHYSICAL DEMANDS: • Essential: Standing, sitting, stooping, bending, walking, finger dexterity, seeing, hearing, speaking.

• Frequent: pushing/pulling usual weight of up to 40 pounds, lifting or carrying up to 30 pounds, reaching above, at and below shoulder level, frequent gripping of an object. • Continuous: repetitive arm/hand motions. • Occasional: kneeling, crawling, climbing stairs, walking on uneven ground, squatting, static gripping of an object for prolonged periods, pushing/pulling usual weight of up to 200 pounds. • Operate pagers, computers, printers, typewriters, telephones, fax machines, dispensers, laminar flow hoods, TPN compounding machines and other pharmaceutical and office equipment.

TYPICAL WORKING CONDITIONS: • Not substantially subject to adverse environmental! conditions. • Subjected to noise/vibration from air conditi! oner, printers, time stamp machines, pneumatic tubes, telephone, etc. • Exposure to hazardous drugs, antibiotics, chemicals, dust, odors and cleaning/ disinfectant fumes.

• Work environment is fast paced, congested and may be stressful due to workload. • Interacts with patients, nurses, physicians and the general public. • Works on multiple tasks at the same time. • May be exposed to communicable diseases.

IV. MINIMUM QUALIFICATIONS A. Education/Certification and Licensure: • Current Hawaii state license as a Pharmacist. • Doctor of Pharmacy degree preferred.

• Post-graduate clinical residency accredited by the American Society of Health-System Pharmacists preferred. B. Experience: • Clinical hospital pharmacist experience preferred. Preferably with experience in rounding with a medical team, monitoring of patient drug therapy, pharmacokinetic dosing and patient counseling and education.
• Experience in unit do! se and IV services preferred. • Ability to communicate professional expertise to all levels of the health care team.
