Additional Job Summary for Clinical Pharmacy Specialist - Oncology II

Provides members with continuous medication treatment where appropriate per authorization or utilizing authorized protocols supplied by software vendors built in the electronic medical record or authorized KPHI clinical pharmacy protocols.

Essential Functions:
Patient-specific Pharmacotherapy

  • Recommends and/or reviews laboratory tests to assess patient response to drug therapy. Identifies medication therapy problems as appropriate.
  • Assists with the design and recommendation of pharmacotherapeutic regimens and corresponding monitoring plans to pres! cribers in way that is systematic, logical, and secures consen! sus from prescriber and patient.
  • Monitors data and modifies pharmacotherapeutic plans as necessary.
  • May perform limited physical examinations, such as blood pressure and glucose measurement, for the purpose of providing medication management services.
Formulary Management

  • Establishes relationships with physicians and facility leaders to successfully partner on drug therapy issues. Balances cost and quality issues.
  • Participates in establishing disease state management protocols in coordination with HPMG physicians, Clinic Nursing, and the Pharmacy and Therapeutics committee.
  • Participates, as appropriate, in development and/or implementation of regional drug initiatives or other formulary management activities.
Consultation

  • Provides patient-specific verbal or written consultations, including pharmacotherapy and pharmacokinetic, to healthcare practitioners.
  • Ensures continuity! of pharmaceutical care and optimal utilization of resources to and from acute and ambulatory patient-care settings.
  • Collaborates with inpatient/outpatient pharmacist or established Regional disease management program(s) for follow-up with high-risk discharges.
Education

  • Designs effective medication-use education for patients and caregivers.
  • Provides one-on-one counseling to patients and caregivers, including information on drug therapy, adverse effects, compliance, appropriate use, and handling.
  • Participates in Regional medication management and formulary education programs to promote appropriate, cost-effective prescribing.
Drug Information Requests

  • Assists with analyzing requestor's drug information need. Employs systematic, efficient, and thorough procedure for retrieving drug information. Follows up consistently on drug information recommendations and assesses effectiveness.
  • P! erforms drug regimen reviews for members as warranted. Assesses patient! s where necessary and communicates information to physician.
Quality Improvement (QI)

  • Participates in monitoring and improving relevant outcome measures and/or quality improvement indicators. Defines QI processes. Develops measures to monitor, assures safety and appropriateness.
Other

  • Provides direct oversight for functions and activities of support personnel to assure responsibilities are performed in accordance with scope of practice and applicable procedures.
  • As needed, participates in counseling patients about the medications used in clinical drug trials.

Qualifications

Basic Qualifications:
Three years as a registered Pharmacist.

Graduate from an accredited pharmacy college recognized by the American Council of Pharmaceutical Education (ACPE).

Valid Hawaii Pharmacist licens! e (must meet education requirement(s) for Hawaii State licensure).

Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification.

Completion of an emergency contraception training course approved by the American Council of Pharmaceutical Education (ACPE), curriculum-based programs from an ACPE-accredited college of pharmacy, applicable state or local health department programs, or programs recognized by the board of pharmacy.

Licensed pharmacists who have completed a PGY1 residency, PGY2 residency or Pharmacy Fellowship or can demonstrate mastery of the knowledge, skills, attitudes, and abilities expected of one who has completed a PGY1 residency.

Demonstrated knowledge of related federal, state and local regulations and accreditation requirements including TJC standards and NCQA Utilization Management standards.

Demonstrated knowledge of and skill in word processing, spreadsheet, database and Inte! rnet PC applications.

Additional Basic Qualifications for Clin! ical Pharmacy Specialist Oncology I:

  • Demonstrated ability to safely handle and/or prepare various pharmaceutical compounds such as chemotherapeutic agents, biologicals or other medications.
  • Demonstrated knowledge of and skill in oral communication, written communication, interpersonal relations, customer service, group process facilitation, problem solving, and conflict resolution.
Additional Basic Qualifications for Clinical Pharmacy Specialist Oncology II:

  • Three years as a registered Pharmacist including two years oncology experience (may include PGY2 residency) focused on the ability to verify chemotherapeutic regimens, dosing, adjusting for organ toxicity, participation in research studies, education of patients and nursing staff.
  • Board Certified Oncology Specialist (BCOP) certification offered by the Board of Pharmaceutical Specialties (BPS) OR successful attainment of BCOP certification within 2 years of! eligibility.
  • Demonstrated mastery of the knowledge, skills, attitudes, and abilities involved in the oncology field.
  • Ability to safely handle and/or prepare compounds such as chemotherapeutic agents, biologicals or other medications.
Preferred Qualifications:
  • Doctor of Pharmacy Degree from a school recognized by the ACPE
Additional Preferred Qualifications for Clinical Pharmacy Specialist Oncology I:

Clinical pharmacy practice experience.

Additional Preferred Qualifications for Clinical Pharmacy Specialist Oncology II:

Clinical oncology pharmacy practice experience.
