(TAMC), US Army Health Clinic S! chofield Barracks, and/or the Warrior Ohana Medical Home which shall contribute to a stable workforce tasked with providing quality health care services to a diverse beneficiary population.
The contractor shall supply personal services for the Department of Pharmacy in support of clinical pharmacy services provided in a variety of patient care environments which may include, but is not necessarily limited to, patient-centered/Solider-centered medical homes, outpatient specialty clinics, inpatient wards, centralized inpatient pharmacy, and outpatient pharmacies.
The contract clinical pharmacists shall perform essentially the same duties as
those required of any active dutypharmacist or Government civil service pharmacist of similar experience and in similar duty assignments.
The contract pharmacists shall provide services including, but not limited to the following:
The contract clinical pharmacist may be responsible for development and participation! in ambulatory care clinic pharmacy practice sites and/or dece! ntralized inpatient practice sites. This shall be accomplished through the development of collaborative partnerships with the medical and nursing staff.
Assess patient’s health care needs. The contract clinical pharmacist will be required to assess patients’health care needs utilizing the following skills.
Retrieve medical, social, family, and drug therapy histories from patients and record this data in the medical record.
Evaluate diagnostic, laboratory, physical assessment, and other clinical information related to disease states and associated pharmacotherapy.
Order laboratory and diagnostic tests as needed to monitor the need for or response to pharmacotherapy.
Perform directed physical assessments to monitor the need for or response to pharmacotherapy.
Determine appropriate pharmacotherapy regimens. Contract clinical pharmacists will be required to determine an appropriate pharmacotherapy treatment plan for individual patients.
Contract ! clinical pharmacists will determine the treatment goals associated with a patient’s established diagnosis and associated pharmacotherapy. Treatment goals will be based upon those designated by the patient’s primary care provider/attending physician or established treatment guidelines.Contract clinical pharmacists will design individualized pharmacotherapy regimens for patients pursuant
to established diagnoses. Unique patient characteristics that influence an individual’s adherence or response to drug therapy will be considered.Contract clinical pharmacists will determine the appropriate measures for monitoring a patient’s response to prescribed drug therapy. Modify pharmacotherapy. Contract clinical pharmacists will modify patients’ pharmacotherapy as appropriate and as defined by the scope of practice outlined by their awarded clinical privileges and approved by their medical and pharmacy supervisory chain.
Contract clinical pharmacists will renew or adj! ust drug therapy as needed to maintain or achieve treatment goals and/o! r minimize adverse medication effects. A patient’s primary care provider or the applicable attending
physician will be consulted for any unusual or complicated cases. Contract clinical pharmacists will initiate drug therapy as needed to achieve treatment goals. Medications may only be prescribed for stable chronic conditions previously diagnosed by an appropriate provider. For unusual or complicated cases, medications will not be initiated before consulting with the patient’s primary care provider or
the applicable attending physician.
Patient education. Contract clinical pharmacists will provide medication and other clinical education to patients as necessary.
Contract clinical pharmacists will assess patients’ understanding of drug therapy and provide medication counseling as needed.
Contract clinical pharmacists will provide medication organizers/calendars, action plans, and instruction to patients when deemed necessary.
Contract clinic! al pharmacists will educate patients regarding appropriate non-drug therapy or preventative measures for established diagnoses or health maintenance.
Documentation. Contract clinical pharmacists will document all direct care patient interactions and consultations in the patient’s medical record as required by institutional regulations and policies.
Contract clinical pharmacists may be required to participate in various clinical pharmacy activities which may include target intervention programs based upon specific medications or other quality indicators relevant to their
practice site, drug utilization and cost effectiveness studies (DUE/DUR), prevention and reporting of adverse outcomes, monitoring for and management of polypharmacy patients, monitoring for controlled substance overuse/misuse, medical and nursing staff education, etc.
Contract clinical pharmacists may participate in or conduct studies involving pharmacotherapy-related issues in the managem! ent of a large outpatient population. Design and conduct studies intend! ed to evaluate particular drugs, their effectiveness and suitability, or to study the reaction of diseases, patients, and conditions to
drug-based treatment.
The contract clinical pharmacist will serve as an expert on pharmaceuticals, pharmacokinetics, and pharmacotherapeutics. He/she will use an extensive and intensive knowledge of the drugs and other pharmaceuticals
available, their nature, action, and interaction to provide authoritative instructions, advice, and assistance to health care providers concerning effectiveness, suitability, hazards, and other aspects of their use. Consultations will be recorded in patients’ electronic medical records when appropriate or requested.
Prepare and conduct or oversee training of physicians, pharmacists, and other health care providers on the use of drugs and other pharmaceuticals. Monitor training of residents, interns, and fellows to insure pharmacotherapy-related issues are fully and correctly dealt with in the! ir curriculum and that the required levels of
skill have been attained.
Recommend or initiate changes and updates to instructions, regulations and other guidance as needed.
The contract clinical pharmacist may be called upon to provide oversight of medication preparation andn dispensing services in an outpatient or inpatient pharmacy when required to meet the overall mission requirements of
the Department of Pharmacy .
The contract clinical pharmacist may be required to process prescriptions, including controlled substance
prescriptions, by selecting the correct drug and dosage form, reducing the drug from bulk to the quantity prescribed,
compounding when necessary, and packaging the drug in appropriate container.
5.1.4.12.2 The contract clinical pharmacist will exercise care in the dispensing of drugs, especially controlled
substances, to assure that legal and agency regulations and instructions are followed, that proper and complete rec! ord
keeping is maintained, and that the drug storage areas are stoc! ked, safe, and secure. He/she will notify the
prescriber, who may be on staff or a community physician, when there are questionable dosages or possible
therapeutic incompatibilities based on a review of the patient’s medication profile. He/she will consult with and
advise nurses and members of the medical staff on matters such as contraindications, side effects, and antagonistic
reactions; and suggest alternative medications as appropriate. Determine the validity of drugs prescribed by the
numerous physician extenders, ensuring the drug prescribed is on the particular physician extender’s list.
5.1.4.12.3 The contract clinical pharmacist will furnish guidance to prescribing physicians on current Pharmacy and
Therapeutics Committee policies, applicable federal and state laws, new preparations, drugs of choice, methods of
compounding non-standard preparations, and special formulas suggested or requested by the professional medical
staff! .
The contract clinical pharmacist will check the work of pharmacy technicians to assure that all drugs dispensed are properly labeled and packaged, and that directions for use and cautions, when necessary, are complete
before the drug is dispensed to the patient. He/she will annotate both prescriptions and prescription labels with initials and/or numbered stamp to show dispensing check has been performed.
The contract clinical pharmacist will dispense prescriptions to patients and counsel them concerning their medications (e.g., uses, side effects, directions). He/she will routinely refer to standard reference texts and journals to keep abreast of current trends, new drugs, and new uses and current drugs.
The contract clinical pharmacist will interpret and evaluate unit dose orders, detecting potential
therapeutic incompatibilities and errors and ensure unit dose orders are picked up from nursing units.
5.1.4.12.7. The contract clinical pharmacist w! ill review and approve technicians’ work. He/she will enter inpatient!
medication orders into computerized or manual patient profiles, place medications into unit dose cassettes, and
deliver them to nursing units.
5.1.4.12.8. The contract clinical pharmacist will interpret sterile product medication orders and detect and resolve
therapeutic and pharmaceutical problems with physicians and other medical and nursing staff members. He/she will
employ appropriate technical skill and/or oversight in the preparation of sterile products which conforms to the
standards established by the United States Pharmacopeia, the American Society of Health-System Pharmacists, and
the TAMC Department of Pharmacy. Sterile products will include parenteral medications, parenteral nutrition, and compounded ophthalmic preparations.
The contract clinical pharmacist will provide drug information to professional staff in a timely manner.
The contract clinical pharmacist will provide technical guidance to and act as a preceptor for phar! macy technicians, pharmacy technician students, pharmacy students, and pharmacy residents when these individuals are
under the contract employee’s professional supervision.
The contract clinical pharmacist will participate in quality assurance activities (e.g. drug utilization evaluations, measures of clinical pharmacists’ impact on drug therapy, performance improvement initiatives, etc).
Education
Must have a Doctor of Pharmacy degree awarded from a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE).
Experience
One year of post-graduate experience as a clinical pharmacist required. Completion of an American Society of Health-System Pharmacists-accredited residency may satisfy this work experience requirement. The Chief, Department of Pharmacy reserves the right to validate work experience as satisfying this requirement. The
Chief, Department of Pharmacy reserves the right to waive th! is requirement for candidates that are otherwise highly qualified.
! Must have an extensive knowledge of the drugs and other pharmaceuticals available at TAMC, their nature, action and interaction.
The ability to analyze patterns and trends to determine appropriate, effective and economical drug therapy management and when improvement in these areas can be identified; the ability to recommend alternatives.
The knowledge to monitor response to prescribed regimens and progress, and to modify dosages or methods or significantly alter regimen without prior approval of physician, but subject to post action review.
Special Requirements/Certifications
Must possess an active, valid, and unrestricted pharmacist license from any U.S. state, the District of Columbia or a U.S. territory.
Be currently trained, certified and maintain qualification in Cardiopulmonary Resuscitation (CPR) as evidenced by successful hands on training and certification in Basic Life Support (BLS) for Healthcare Providers through an American H! eart Association approved training program. On-line courses will not be accepted.
