Medical Home Port Clinical Pharmacist Services Headquarters, Naval Health Clinic Hawaii BHC Makalapa (1 FTE) and BHC Kaneohe Bay (1 FTE) Site of Service. The healthcare workers (HCWs) shall provide services in support of the Naval Health Clinic Hawaii (NHCH). Services shall be provided at the Headquarters (HQ), NHCH Branch Health Clinic (BHC) Makalapa and BHC Kaneohe Bay HI, or associated BHCs/Annexes within a 25mile radius of the NHCH. No mileage or per diem shall be paid to the contractor for local travel.

The HCWs shall be required to participate in an integrated medical healthcare team. The HCWs providing service under this Contract/Task Order (TO) will be supporting the Navy's Medical Home Port program. The objective of this program is to enhance continuity, wellness and disease management for the patients. Clinical Pharmacists, Ambulatory Care (2 individual FTEs) Qualifications.

The HCWs shall possess and maintain the minimum qualifications stated in Section C of the basic contract. PHARMACIST Have a bachelors degree in Pharmacy or Doctor of Pharmacy from a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE). Possess current, valid, unrestricted license to practice Pharmacy in any one of the fifty States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.

Have working knowledge of pharmacy computer systems such as AHLTA. In addition, the HCWs shall possess and maintain the following qualifications: Possess a Doctor of Pharmacy (PharmD) degree from a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE) and have demonstrated competency in medication therapy management. Competency in medication therapy management may be demonstrated by possessing at least one of the following: * Successful completion of a residency program approved by the American Society of HealthSystem Pharmacists (ASHP) or the American College of Clinical Pharmacy (ACCP). * Specialty Board Certification in either Pharmacotherapy or Ambulatory Care through the Board of Pharmaceutical Specialties (BPS).

  • A minimum of 3 years of documented Pharmacist experience within the last 5 years in the clinical ambulatory care setting. Provide two letters of recommendation, written within the preceding 3 years, from supervisors, physicians or administrative supervisors attesting to the HCWs clinical/professional skills and competencies. Letters must include; name, title, and phone number, date of recommendation, address and signature of the individual providing the recommendation. Staffing and Scheduling.
The contractor shall provide two individual fulltime Clinical Pharmacists. To maintain continuity of services, the HCWS MUST BE FULLTIME INDIVIDUALS. The Commanding Officer will not approve more than one individual for each required position. The Commanding Officer or designated Government representative will supervise the HCW.

The HCWs shall normally provide services Monday through Friday, excluding the day of observance of Federal holidays, between the hours of 0700 1800. The HCWs shall normally be assigned an 8.5 hour shift to include a 0.5 hour uncompensated lunch break. Each HCW shall be assigned 40 hours of service per week. The specific schedule will be prepared approximately 30 days in advance.

Each HCW shall not exceed 80 hours of service per two week period. Any changes in the schedule shall be coordinated between the HCW and the Government. The HCWs shall comply with Defense Medical Human Resources System internet (DMHRSi). HOLIDAYS AND LEAVE.

The HCWs shall accrue 8 hours of personal leave (annual plus sick) for each 80 hour pay period and shall be subject to leave approval in accordance with Section C of the basic Contract. The HCWs shall be compensated, as applicable, for federal holidays in accordance with Section C of the basic Contract. Duties. PHARMACIST Perform a full range of pharmacist procedures.

Deliver comprehensive pharmaceutical services within the personnel, supply, and equipment capabilities of the facility. Counsel patients regarding appropriate use of medications. Review patient profiles at the time of dispensing in order to closely monitor medication therapy. Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients.

Intervention with providers shall be made for noted discrepancy of above listed items. Compound medications as required. Ensure accuracy of all pharmaceutical products prepared or processed by technical staff. Work closely with clinical staff in developing patient wellness programs such as smoking cessation programs and asthma classes.

Provide medication information to the clinical staff as required. Provide medication and health care related in service training to clinical staff on new medications on the market; appropriate use of medications; comparison of current medication therapies; and other topics as deemed appropriate by the department head. Provide clinical direction of corpsmen and pharmacy technicians and provide input into job performance evaluations. Maintain inventory control of all pharmaceuticals in the pharmacy, especially scheduled pharmaceuticals, and ensure pharmacy security.

Maintain appropriate records and security of all scheduled controlled substances according to applicable regulations. Consult with other specialty practitioners who have been referred for pharmaceutical services. Ensure that work areas are clean and safe according to applicable regulations. Document significant medication interactions, and pharmacy interactions with prescribers.

Execute drug utilization reviews (DURs) as deemed necessary. In addition to the duties listed in Section C of the basic Contract, the following duties shall be performed: Provide Medical Therapy Management (MTM) services to eligible adult and pediatric patient populations for medical conditions that may include anticoagulation, diabetes, asthma, hypertension, hypercholesterolemia, and chronic pain. Coordinate and collaborate with medical healthcare team members to assist with medication review to provide health maintenance and preventative services to individual patients as required.
